Please forward any pertinent medical records, including office notes, blood test results, ultrasounds and test results from the last two years from your primary physician’s office. Providing this information in advance will ensure that your first appointment is as efficient as possible. To save you money and avoid unnecessary testing we do not order lab work prior to your consultation. | |

| | What should I expect during my first office visit?
Your initial consultation is approximately one hour and entails a review of your medical records and medical history and a thorough evaluation of your treatment to date. We will discuss your treatment options, order relevant lab tests and initiate a treatment plan. | |

| | What should I expect as follow-up from my first consult?
Follow-up appointments are scheduled as needed to evaluate your symptoms, your response, review any labs and adjust your regimen. The frequency of follow up visits will depend on what treatment you are on and how you feel. If you live out of town we are happy to schedule your follow up by telephone appointment.

Following your consultation with Dr. Greene you will have the opportunity to speak with a financial consultant to review the fees associated with your treatment as well as the payment options available. | |

We do not sell products or supplements from our office. We prefer our relationship with you to be free of financial conflicts of interest. Numerous supplements lack scientific validity, some may be harmful and many are expensive. If you are currently taking any supplements we will be happy to discuss their safety and effectiveness with you. | |
